The permission form is enabled for fill in on line prior to printing. Permission forms require signatures- which may be digital signatures or handwritten.

First, right click on the permission form link and save it to your desktop.  Then open the form and complete as much as possible by typing.  DO NOT PRINT A BLANK FORM AND FILL IT OUT BY HAND.  Save a copy of the form to your computer with all your information in it - and then attach it to an email.  We accept forms back as emails, faxes or by postal mail.

PLEASE FILL IN THE FORMS BY TYPING and not handwriting. WE MAY REFUSE TO ACCEPT FORMS THAT ARE ALL HANDWRITTEN IF THEY ARE NOT LEGIBLE.

You will need an updated version of Adobe Reader.  If you have an older version of Adobe Reader, you will get an error message.
